1. ENTRY: No purchase necessary. You must be thirteen (13) years or older at the time of entry to be eligible for this contest. To enter, complete the official entry form that is located at http://www.fanaticssweeps.com/12Weeks. We will ask you to provide your first and last name, email address, telephone number, and mailing address. The official online entry form must be used. Email or other entries will not be accepted. The entry form box labeled, "I am at least 13 years of age or older and I have read and agree to the terms and conditions,” must be checked for participation in this contest. Each participant may enter one (1) time per day during the Promotion Period. Multiple participants are not permitted to share the same email address. Any attempt by any participant to obtain more than the stated number of entries by using multiple/different email addresses, identities, registrations and logins, or any other methods will void that participant's entries and that participant may be disqualified. Use of any automated system to participate is prohibited and will result in disqualification. Once submitted, a confirmation email from info@fanaticssweeps.com will be sent directly to the registered email address of the potential winner. To ensure proper delivery, the potential winner should add info@fanaticssweeps.com to their contact list. The 12 Weeks of Summer Sweepstakes begins at 1:00 p.m. EST on June 1, 2009 and ends at 5:00 p.m. EST on August 28, 2009. One (1) winner will be selected each week, of the Promotion Period. Winners will be contacted via email and listed on the winner’s list.

2. PRIZE: Twelve (12) prizes available. Prizes cannot be exchanged for cash. Prizes will be shipped to the home address of the potential winner. However, before shipping, the potential winner will be contacted via email to confirm the shipping address. This communication will be sent from info@fanaticssweeps.com. To ensure proper delivery, the potential winner should add info@fanaticssweeps.com to their contact list. If a potential winner does not respond to email communication or cannot be reached by telephone confirming that the prize was received within five (5) days after notification was sent, or in the event the potential winner does not properly claim his or her prize in accordance with these Contest Terms and Conditions, the prize will be forfeited. In the event any potential winner does not comply with these Contest Terms and Conditions for any reason, that potential winner will be disqualified and an alternate potential winner will be selected at random. No prize transfers or cash alternatives permitted. All decisions of the Sponsors or their designees are final in all matters relating to this Giveaway. Return of any prize/prize notification as undeliverable may result in disqualification and selection of an alternate winner.

ELIGIBILITY: Except as otherwise prohibited herein or by applicable law, this Contest is open to individuals that are at least thirteen (13) years of age or older as of entry date and reside in the 48 continental United States. In order to be eligible for prizes, we must be able to contact the winners via the email address or telephone number provided on their entry. The winner agrees to allow Surf Fanatics to post his or her name on our Web site, or in other material, in conjunction with the announcement of their winning. Any contestant who cannot be reached at the email or telephone number provided, or who will not agree to allow his or her name to be announced as a winner on any Surf Fanatics Web site, will not be eligible to receive prizes.

4. ODDS OF WINNING: The odds of winning depend on the total number of eligible entries received during the entry period.

5. TAXES, EXPENSES AND OTHER CONSEQUENCES OF WINNING: Income or other taxes on prizes or other expenses related to the acceptance of a prize are the sole responsibility of the recipient. By accepting, or agreeing to accept a prize, the winner acknowledges, understands and agrees that he or she, and any individual benefiting from such prize, is and/or are solely responsible for the consequences and implications that may arise in connection with any other obligations or agreements by which he or she may be bound.
As a condition of receiving this prize the potential winner agrees that the Sponsors and their designees may use their names, voices, photographs, likenesses, addresses (city and state), biographical and company information, prize information and/or statements about this giveaway for promotional, marketing, advertising and publicity purposes in any and all media worldwide, in any form and format, at the discretion of the Sponsors, without further notice, nor any limitation, restriction or compensation unless otherwise prohibited by law.

A decision regarding the contest winner is final. You must agree that, regardless of the outcome, you will not challenge the decisions of Surf Fanatics or file any claims or suits against Surf Fanatics, their affiliates, directors, officers, or employees, arising from the conduct of the contest. You agree that Surf Fanatics may, in its sole discretion terminate this contest without any liability to you. Surf Fanatics reserves the right to alter any rules of any contest at anytime.

9. AUTHORITY TO REGISTER AND/OR TO ACT AS AGENT: You represent and warrant to Surf Fanatics that you have full legal authority to enter this contest on the site. In addition, if you are entering third parties, you represent and warrant that you have been duly authorized to act as agent on behalf of such parties in completing this entry. By proceeding with this contest entry, you agree that the terms and conditions of this contest shall apply equally to you and to any third parties for whom you are acting as agent.

COMPLIANCE WITH CHILDREN'S ONLINE PRIVACY PROTECTION ACT (COPPA): You represent and warrant that, in compliance with COPPA, you are over thirteen (13) years of age, and that if you are entering a child under fourteen (14) years of age you are the parent of such child, and do hereby consent to the collection of such child's personal information by Surf Fanatics.

APPLICABLE LAW; CONSENT TO JURISDICTION: The Surf Fanatics site, as well as its affiliate's sites (excluding linked sites) is controlled by TeamFanShop from its offices within the state of Florida in the United States. By entering this contest, both you and Surf Fanatics agree that the statutes and laws of the state of Florida, without regard to the conflict of laws principles thereof, will apply to all matters relating to this contest, or other use of the Surf Fanatics site. You agree that exclusive jurisdiction for any dispute with Surf Fanatics resides in the courts of the state of Florida and you further agree and expressly consent to the exercise of personal jurisdiction in the courts of the state of Florida in connection with any dispute including any claim involving Surf Fanatics or its affiliates, subsidiaries, employees, contractors, officers, directors, telecommunication providers and content providers. This contest may be subject to additional federal, state or local laws. The winners are responsible for any applicable fees or taxes.

12. SEVERABILITY: If any provision of these terms and conditions shall be unlawful, void, or for any reason unenforceable, then that provision shall be deemed severable from these terms and shall not affect the validity and enforceability of any remaining provisions.
